If a third party scholarship or external scholarship agency requires the University to complete a form as a condition of your scholarship, please submit the form to the Office of Student Financial Aid Services (OSFAS) by email at financialaid@uconn.edu or by fax at (860)486-6629.  Please allow three to five business days for processing.
